Requesting Your Health Records

Patients may request access to their health care records by submitting a written request to Basileia Counseling PLLC.

Requests can be submitted by:

Or completing a written records request form upon request

Requests will be processed in accordance with applicable state and federal laws.

Filing a Consumer Complaint

Consumers have the right to file a complaint with the Office of the Texas Attorney General if they believe their rights regarding health records access have been violated.
